    —Individual plans with effective dates starting April 1, 2009
    —A small group plan (2-50 employees) that has been renewed on or after July 1, 2009
    —MinnesotaCare, Prepaid Medical Assistance Program (PMAP) or Prepaid General Assistance Medical Care (PGAMC)
    —SecureBlue (MSHO) and CareBlue (SNBC), for drugs covered by Medical Assistance
